Renal Transplant Coordinator
Job Summary: The Renal Transplant Coordinator’s role involves coordinating patient evaluations, facilitating communication among healthcare providers, and ensuring that all aspects of the transplant process are efficiently managed. The coordinator also plays a key role in patient education, pre- and post-transplant follow-up coordination, and compliance with regulatory requirements.
Key Responsibilities:
- Patient Management & Continuation of Care:
- Coordinate the care of patients from initial evaluation through post-transplant follow-up.
- Schedule and coordinate patient appointments, tests, and procedures.
- Ensure timely and accurate documentation of patient information and medical records.
- Ensure proper documentation and tracking of donor organ matches and transplant procedures.
- Education and Support:
- Educate patients and families about the kidney transplant process, including pre-transplant preparation, surgery, and post-transplant care.
- Provision of support for all families, potential donors and recipients during the live donor work-up
- Assist with the organization of all aspects of the live donation process
- Maintain highly effective communication among all members of the multidisciplinary team
- Oversee all aspects of the workup process for renal transplantation
- Ensure relevant status of the patient on the transplant waiting list
- Address any patient or family concerns and guide managing expectations.
- Ensure compliance with all regulatory requirements and transplant centre protocols.
- Maintain up-to-date knowledge of transplantation policies and procedures, including organ allocation and eligibility criteria.
- Ensure proper documentation and tracking of donor organ matches and transplant procedures.
- Carry out regular audits of renal transplants
Qualifications:
- Education: A bachelor’s degree in medicine or nursing (additional degrees in healthcare administration or related fields will be preferred). Experience in Renal Transplantation or nephrology will be preferred.
- Experience: Minimum of 4 years post-graduation experience in a healthcare setting, with a year in nephrology or transplant care. Experience as a transplant coordinator or in a similar role is highly desirable.
- Skills:
- Strong organizational and multitasking abilities.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with a multidisciplinary team.
- Knowledge of renal transplant procedures, organ procurement, and regulatory requirements.
- Proficiency in electronic medical records (EMR) systems and other healthcare technologies.
Work Environment:
- The role typically involves working in a hospital or clinical setting, which may require occasional on-call duties or irregular hours.
